District Court Pro Se Dissolution Forms
These forms are prepared for your use in obtaining a divorce when you have not retained an attorney to represent you.

| B | Joint Dissolution with Children | If you and your spouse agree to jointly file for a divorce, and you have minor children |
| C | Dissolution without Children | If you are seperately filing the divorce petition and do not have minor children. |
| D | Response to Dissolution Without Children | If you are responding to a divorce petition filed by your spouse, and do not have minor children |
| E | Dissolution with Children | If you are filing the divorce petition and you have minor children |
| F | Response to Dissolution with Children | If you are responding to a divorce petition filed by your spouse, and you have minor children. |
| G | Default Packet - Dissolution | If you have filed a divorce petition in Packet C or E and your spouse has not responded and you want his/her default entered |
| H | Petition for Permanent Parenting Plan | If you are not married to your child(ren)'s co-parent and you want to establish a parenting plan. |
| I | Response to Petition for Permanent Parenting Plan | If you are responding to a petition for a parenting plan which was served to you |
| J | Default Packet - Parenting Plan | If you have filed a petition for a parenting plan in Packet H and the co-parent has not responded and you want his/her default entered |
| L | Joint Petition for Permanent Parenting Plan | If you are not married to your child(ren's co-parent -- and you both agree and what you want for a parenting plan -- and want to have it on record with the Court. |
